To register for an Ohev Shalom Youth Group, complete the form below and press Submit.  Please send a check for the Youth Group fees to Ohev Shalom (see below).  You may also print out this page and mail the completed form with your check to Ohev Shalom.

Fees are for Ohev Shalom congregational families. If your family is not affiliated with Ohev Shalom, an additional $18 has been added to the fee.  (All fees include regional and international dues)
